001C and Guidelines posted on web

Form 001C is available on the net: Go to http://ceowestbengal.nic.in , click on the link "Forms", then on "Photo I-Cards" to go to 001C

Sample guidelines in Bengali for Designated Officers for Summary Revision 2008 are posted on a separate link button on bottom left called "Summary Revision 2008". Just click on it.
Two caveats: Few dates may require changing if revised schedule comes into effect. (2) The draft is to be modified (and translated into Nepali for hill subdivisions of Dj) as per local variations.

From the Bengali guideline, & 001C from, posted in the website,
some points remains to be cleared.
1)Whether use of 001A along with form 6/8/8A is not permissible this yr., besides 001C ?
2)In part-II of 001C there is provision of mentioning part/ sl. no. of elector. For new entrants in a part, whether any sl. no. should be assigned by ERO or be left blank?
3)Obviously there is no need for affixing photo in corr. Forms 6/8/8A, this yr. – for confirmation pl.
4)Has any form devised for listing of such 001C forms recd. in designated locations(with photo) & in DPL (Block Office) on photography date, for keeping track, part wise?
5)Bengali version of 001C form is not available in the web till now. When will it be available?

1) Don't use 0001A because it does not have any mention of date of birth

Thanks for Form 001C in Bengali. One minor mistake: Just above the signature of the elector, where there is a N.B, the guideline should be"Uparillikhita" rather " Nimnalikhita".

Minor spelling mistake: Where ERO's representative is making comments about verification of Photo and DOB, Bengali spelling of "Ha(n)" is misspelt.

With the use of 00IC henceforth,should
1)the EPIC related forms.001.002.003.004 used in the PEC be discontinued?
2) is the photo required in the declaration form for students studying outside(to be submitted along with Form 6)?

Now to respond to your queries:

1.001C is meant to be a multipurpose EPIC form as it contains not only provision for photographs but ALSO date of birth. So, in effect, this replaces 001 -4 forms BUT since it is not a statutory form, no application may be rejected merely because a wrong form has been used.
